How far in advance do I need to call to schedule an appointment?
In keeping with our philosophy of fulfilling our patients needs, same day appointments are regularly available.

How long will I wait in the office?
Our dedication is on you, the patient, and thus we do everything in our power to see you at your scheduled time.

What do I do to make an appointment?

What if I am unable to keep my scheduled appointment?
We understand that life is hectic and that this problem will arise from time to time. However, we kindly ask you to provide us with 24 hours notice in order to schedule another patient in need.

Does health clinic accept my insurance plan?
We do our best to work with the patients we see and because of that we accept insurance plans

What if I do not have medical insurance?
In the current cultural climate we understand that this is a possibility. We do our best to keep your costs a minimum, so the first visit $110, follow up $70. If blood work is necessary there will be an additional nominal fee. We accept cash and all major credit cards.

Where is the office located?
Our offices are conveniently located in midtown Manhattan only two blocks away from the Grand Central Station

What is required prior to my office visit?
You will be asked to fill out a few forms and provide us with your contact info and insurance status. It is quick and easy.

Who is the doctor?
We are proud to have Dr. Fuzayloff on staff. He is a board-certified primary care physician with many years of experience who dedicates himself to combining high quality medical care with a warm and caring environment. His professional manner will have you feeling like a member his immediate family!

To whom does Dr. Fuzayloff refer patients if he needs a specialist?
Dr. Fuzayloff refers his patients to only the best and most qualified specialists. Over the years Dr. Fuzayloff has developed personal relationships with New York’s top specialists and around the country. Feel confident that you are in the best hands at the Midtown Medical Clinic

How can I leave a message for Dr. Fuzayloff?
Simply call 212-696-5900 and leave a message with a secretary. Since Dr. Fuzayloff is often busy seeing scheduled patients during the day, but your calls are of the utmost importance to him and will be returned within a few hours.

How will I learn of the test results that the doctor has ordered?
Most common lab or x-ray studies are available for review by Dr. Fuzayloff within five business days. After a review of the result Dr. Fuzayloff will discuss the results with you over the phone.
